- Check your email. You can check your email on your computer or you can open your email on your phone and complete them on your device.
- You will receive an invite to take to submit your intake forms with a password. Your password will be random letters to access the forms.
3. Once you enter your password, your will be able to complete your patient intake. Please note that there are three forms that will need electronic signatures. After you read and sign the forms, you can then complete your basic information and health history directly below.
4. Once you complete your information, click submit forms and then you are completed. The forms will be sent directly back to us. If you have any questions on the forms, don’t hesitate to call us.
